Sweden is the largest single MedTech market in the Nordics, worth approximately EUR 2.0–2.2 billion annually and home to global champions like Getinge, Elekta, Mölnlycke and Arjo. For a foreign manufacturer, the attraction is scale, sophistication and predictability — but also the complexity of 21 regional authorities (regions), each running its own procurement, each with its own clinical governance, each signing its own multi-year tender contracts.
Regulatory oversight sits with Läkemedelsverket (MPA) in Uppsala, which acts as Sweden's competent authority for EU MDR 2017/745. Post-market surveillance, vigilance reporting and economic operator registration all go through the MPA's eServices portal. Swedish clinicians are early adopters of evidence-based MedTech and demand clinical data, real-world outcomes and strong post-sales training — distributors without clinical specialists rarely win.
Most hospital procurement runs through regional tenders organised under the Public Procurement Act (LOU), with some joint procurement coordinated by Adda (formerly SKL Kommentus). Läkemedelsverket (the Medical Products Agency, MPA) is the competent authority for medical devices under EU MDR and IVDR. Sweden follows EU rules directly, so a valid CE mark issued by an EU notified body is accepted without additional national approval.
Public procurement is decentralised to 21 regions, each running its own tenders under the Public Procurement Act. Tender cycles typically run 2–4 years with options for extension. Adda coordinates some joint procurement. Winning a regional tender secures volume but compresses margin; losing it typically shuts you out of that region until the next cycle. Yes, if your product is CE-marked and you appoint a Swedish or EU authorised representative and a Swedish distributor or importer who handles economic operator obligations. Many foreign OEMs enter via distributor agreements without establishing a Swedish entity. A local entity becomes worthwhile at higher volumes or when you want direct clinical engagement and account control.
